Player Eligibility

Youth player eligibility is determined by player age as of January 1st, 2009. Proof of age required
upon request.

• $350 per team, payable by cash, debit, VISA, Mastercard. Accepted at our BHI WELLAND Location ONLY.
• Roster and payment must be submitted no later than Saturday July 4th, 2009.
• All youth players MUST HAVE a helmet with facemask, elbow pads, hockey or ball hockey gloves, stick
with ABS or composite blade(No tape allowed), shin pads, jock or jill strap, and appropriate running
shoes.
• Adult players MUST HAVE gloves and stick in good condition with no tape.
• Each team MUST wear matching shirts properly numbered. Numbered pinnies are available at BHI at no
cost if required.
• Alcohol is strictly prohibited on BHI properties.
• BODY CHECKING WILL NOT BE TOLERATED AS PER BHI RULES.
• FIGHTING IS ABSOLUTELY FORBIDDEN AT ANY TIME DURING OR IN-BETWEEN GAMES – Consequence is
Major Penalty PLUS Suspension from remainder of tournament.
• Waiver Forms MUST be completed by parent or legal guardian if under the age of 18.
• Play will be governed by BHI Rules and Guidelines.
• Part of proceeds go to Boston Pizza Foundation Artificial Pancreas Fund in support of the
Juvenile Diabetes Research Foundation.

Space is limited! Register your team early to secure your place in the tournament. Teams will be accepted
in the order of their registration date until their division is full.
OUT OF TOWN TEAMS WILL NOT PLAY ON THURSDAY JULY 16th or 23rd, 2009.
